1. सोसायटी द्वारा पारित संकल्प: बोर्ड संकल्प (Board Resolution) Download Board resolution template
2. सोसायटी का पंजीकरण प्रमाणपत्र (केंद्रीय/राज्य/जिला रजिस्ट्रार द्वारा जारी)
3. संस्थान के नाम और संबंधित बैंक खाते के विवरण के साथ रद्द किया गया चेक या पासबुक की प्रति
4. जीएसटी और पैन प्रमाण पत्र
5. सदस्यता के लिए अतिरिक्त सूचना पत्रक भरा गया (Information sheet for Membership): Download Template

ऑनलाइन आवेदन जमा करने के बाद:

1. ऑनलाइन जमा किए गए उपरोक्त सभी दस्तावेजों की मूल प्रति (Original) अपने पास रखें।
2. आवेदन की स्वीकृति की पुष्टि प्राप्त होने पर, आवेदन पत्र के साथ संलग्न कर सभी भौतिक दस्तावेजों (Physical Copy of Document) को ईमेल में दिए गए पते पर कूरियर के माध्यम से भेजें।

Important Notice: For a successful submission of the membership application form, please ensure that the following documents are ready in PDF format:

1. Resolution passed by the Society: Download Board resolution template
2. Registration certificate of the Society (issued by the Central/State/District Registrar)
3. Cancelled cheque or passbook with the name of the institution and relevant bank account details
4. GST and PAN certificate
5. Filled in additional Information sheet for Membership: Download Template
6. If you want to fill the form offline then please fill this form. Download Form

After submitting the online application:

1. Keep the originals of all the above documents submitted online.
2. Upon receiving the confirmation of the acceptance of the application, send the physical documents via courier to the address provided in the confirmation email.
